What we collect, why we collect it, and what you can ask us to do with it.

Last updated 31 August 2026. This policy explains how EvoHealth handles your information, both as a patient of the clinic and as a visitor to this website.

Information we collect

When you contact the clinic or fill out a form, we collect what you give us: your name, phone number, email address, and whatever you tell us about your symptoms and history. As a patient, we also collect the clinical information that care requires, including your medical history, intake questionnaires, lab results, medications, supplements, and notes from your visits. If you use a wearable or continuous glucose monitor as part of your care plan, we collect the readings you choose to share.

Health information and HIPAA

Your medical record is protected health information, and it is handled under HIPAA. That means we use and disclose it to treat you, to run the clinic, and to get paid for care, and otherwise only with your written authorization or where the law requires it. We do not sell your health information, and we do not use it for advertising.

How we use what we collect

Clinical information is used to evaluate you, order testing, build and adjust your care plan, and follow your progress over time. Contact information is used to schedule you, send results and plan updates, and answer your questions. If you ask to hear from us about clinic news, you can stop those messages at any time without affecting your care.

Telehealth visits

Telehealth appointments run on a platform that supports HIPAA requirements, and the visit itself is documented in your chart the same way an in person visit would be. Visits are not recorded unless you are told and you agree. Please join from a private space and a connection you trust, since we cannot control the security of your own network or device.

Labs, pharmacies, and other partners

Care often involves outside parties: reference laboratories that run your panels, phlebotomy sites that draw your blood, compounding and retail pharmacies that fill prescriptions, and the electronic record, scheduling and billing systems the clinic uses. Each receives only the information needed to do its part, and each is bound by its own privacy obligations.

Payment information

EvoHealth is a direct pay clinic and does not bill insurance. Card, HSA, and FSA payments are processed by a payment processor, and full card numbers are not stored by the clinic. If you ask for a superbill to submit to your own insurer, it will include the diagnosis and service codes that insurers require.

This website

This site uses cookies for basic functionality. Where website analytics are in use, they measure in aggregate which pages are read and where visitors arrive from, and that data is never used to build a health profile about you or to target advertising to you. Most browsers let you block or delete cookies, and the site still works if you do. Forms on this site are for scheduling and general questions, so please do not send detailed clinical information through them.

Your rights

You can ask for a copy of your record, ask us to correct something that is wrong, ask for a list of certain disclosures, and ask us to communicate with you a specific way. You can also withdraw an authorization you previously gave, going forward. Requests can be made by phone or in writing, and we will tell you if any request cannot be granted and why.

How long records are kept

Medical records are retained for the period Kansas law and professional standards require, and longer when there is a clinical reason to keep them. Records are stored in access controlled systems, and staff see only what their role requires.

Changes and questions
